What is misophonia, how is it different from sound sensitivity - and why do family dinners drive you mad
It's family dinner. Your mother is sitting across from you.
Five minutes in, you can already feel it:
Your face - tense. Your breathing - shallow. Your body - somewhere between frozen and about to explode.
She's chewing. The others at the table don't even seem to notice. In your ears, it's deafening.
And in this moment, one thought is running through you:
I need to get out of here!! And we're only on the starters.
Maybe you know this feeling. Maybe not with your mother, but with someone - a colleague at lunch, at a friend's place, the person next to you on the train. A sound that drives you up the wall while everyone else around you doesn't even seem to register it.
You've wondered whether you're highly sensitive. Or just too tense. Whether the fault lies with you - that your brain is too easily rattled, too intolerant.
The good news:
There's nothing wrong with you. You're not just being difficult. Your brain isn't broken.
The slightly less easy news:
These intense reactions to certain sounds - they can be influenced. But changing them takes more work and patience than "just think positive" wants to sell you. But - and this is important - often more is possible than you might be able to imagine right now.
Because it's not the sound alone. This unbearable restlessness, this I have to get out of here, is strongly tied to the story your nervous system has stored about this sound. And that story is something you can rewrite - at least in parts.
The same sound, two completely different reactions
Your kid is eating chips. Mouth open, crumbs everywhere. Crunching, munching, savoring every bite. You hate this sound. But it's not unbearable.
Why? Because deep down you know: They just don't know better yet. They can still learn.
Hope. Possibility. Future.
Then the person you love is sitting next to you. Chips again, mouth open again. You've explained it 1,000 times. You've told them how bad this is for you. And now - for what feels like the 1,001st time - they're chewing those chips just as loudly, in exactly the same I don't care how this feels for you kind of way.
And when you bring it up, what you get is: "Is that getting on your nerves again? You're really overreacting." As if their own enjoyment of the chips obviously mattered more than your feeling that this sound is tearing you apart inside. And tomorrow it'll be "You mean everything to me" again (just don't let your needs ruin my enjoyment of the chips).
That's the double message: "Let's spend time together because we matter so much to each other - but how you experience that togetherness had better not spoil my mood."
When sounds work on you like this, they become unbearable. Your heart races. You just want to leave. Your brain has already run the numbers: "This person doesn't respect my boundaries. My needs and I don't really matter here." And maybe you also fight that conclusion - because the affection they do give at times still feels too good to risk by setting firmer boundaries.
That's the difference:
It's not just the volume of the sound. It's not simply that your brain is "too sensitive."
What hits so hard is the boundary violation - and the double message behind it.
With your kid: no deliberate boundary crossing yet. Your brain relaxes (relatively).
With the person by your side: negligent disregard for how you're doing - despite all the nice words.
The sound becomes a symbol. It's no longer just about chewing - it's about power, about authenticity, about: Do you actually see me, or only when it suits you?
What's happening in your brain
Your brain has one central job: keeping you safe. That's why a "scanner" is always running in the background, checking: Am I safe? Is this a threat to me?
If you were young and your boundaries were regularly disrespected - whether through emotional neglect (not seeing you, not being present) or deliberate disregard (they knew, and they didn't care) - your nervous system learned that and stored it away.
If you learned as a child that your boundaries don't count - that asking for respect gets ignored or answered with anger - then your nervous system learns: danger lives here. Maybe not physical danger, but emotional danger.
Your nervous system then had to learn to scan early:
What kind of mood is this person in? Are they going to cross my boundary? Can I protect myself in time?
This constant scanning is called hypervigilance - it's as if your brain were running a permanent security check. And that costs energy: the scanning itself, and being perpetually ready to fight, flee, or freeze. Your nervous system is operating in survival mode, not rest mode.
Years later, the person you're with is sitting next to you, loudly chewing chips. To their brain: a normal sound. To your brain: a boundary violation in miniature - this person could do it differently, but they don't. And that trips your old alarm: "This person doesn't take my boundaries seriously. I need to protect myself."
That's not paranoid. That's you noticing an understandable alarm - one that, given your history, was even necessary.
And here's where it gets interesting: this anger, this reaction - it isn't just a problem you need to "overcome." It's also a signal about your own needs.
The anger tells you: "Stop. This isn't okay. My boundaries need defending." That, in itself, is healthy.
Anger gives you the energy to stand up for yourself.
A problem arises when that anger can't or isn't allowed to fuel your actions. When you had to learn to swallow it down to avoid being punished for voicing your boundaries. Sometimes it flips into the opposite, and the anger floods you so completely that you have no options left - except leaving the room or suffering in silence.
And here's the strange part: this alarm runs mostly below your conscious awareness.
All you feel is the aftermath - the rising anger, the restlessness, the urge to run.
Your brain isn't simply too sensitive or too intolerant.
It became highly reactive out of necessity - it had its reasons.
But when your kid eats chips, something different tends to happen: your prefrontal cortex - the region that thinks and evaluates rationally - switches on. You know: the kid can't help it yet. This isn't a deliberate boundary violation; it's undeveloped motor skills, missing knowledge, no bad intent.
Your amygdala (the brain's alarm and emotion center) settles down again (at least a little).
The same sound, but the emotional evaluation is different.
That's the crux: often, it was never just the sound. It was also the story behind it - and whether you believe "this person could change it but doesn't" or "this person simply can't yet."
One important note: your automatic reaction is trainable
Before we go on: yes, these neurological patterns are trainable. Your brain can learn new stories about sounds. But it's not linear - meaning: not always moving forward, not "it gets better every week."
Your new freezer hums. But you're thrilled to have it. So you can tolerate that sound, at least in good phases - or even tune it out. The positive emotional evaluation allows your brain to be less reactive. After all, that thing is storing frozen pizza and ice cream for you.
But it's also phase-dependent. On exhausting days, when your emotional energy is low, when you're already overstimulated - even that beloved freezer can become a burden again.
That's not failure, as in: "Great, I failed again at mindfully breathing my feelings away."
That's neurobiology in real life. Your nervous system works with the resources you have right now.
And some days are simply emptier - and therefore harder - than others.
Misophonia vs. sound sensitivity:
what's the difference?
Maybe you're asking yourself at this point: is what I have "just" sound sensitivity - or already misophonia?
These terms get mixed up a lot - understandably, because the line between them is blurry.
Sound sensitivity is a generally heightened perception of ambient sounds. Some people simply perceive sounds more intensely - that can be genetic, can be linked to neurobiological differences (as in ADHD or autism), but can also be a consequence of chronic stress. The reaction is usually more like: this is too loud, too intrusive, I need to filter it out.
Misophonia is more specific. It's not about all sounds, but very particular ones: chewing, sniffling, spoons scraping plates, certain breathing sounds. And the reaction is intensely emotional - not just "that's loud," but: Rage. Disgust. The urge to flee. Sometimes hatred toward the person making the sound.
The difference often lies in the fact that misophonia is coupled with an emotional evaluation - especially the perception that this person could do it differently, but doesn't.
Important:
this coupling runs unconsciously. So you can also be triggered by the chewing of people you don't blame for anything at all - that doesn't make your reaction any less real, and it doesn't make you a bad person.
And then there's hyperacusis - which I know firsthand, because I've been diagnosed with it. Hyperacusis is a pathologically heightened perception of loudness, where sounds in general - regardless of context - feel unpleasantly or even painfully loud. I can hear frequencies many other people can't perceive at all: the hum of the electricity in the walls, for example. For me, that means my hearing simply hands my nervous system more material for emotional evaluations to attach to. Hyperacusis and misophonia can occur together - misophonia was actually first described in hyperacusis patients.
But: not everyone with hyperacusis develops misophonia, and not everyone with misophonia has hyperacusis.
None of this means that emotions play no role in sound sensitivity. But in misophonia, the emotional layer is the central - and often unconscious - driving force.
And that's the good news:
if emotional evaluation is the lever, then that lever is something you can work with.
What awareness alone can already change
Now you're probably thinking: okay, I get the theory - but what does it actually do for me?
Here's what it does: once you know which emotional story sits behind your reaction, you don't automatically lose the reaction - but you gain something else: choice.
An example: you live in an area with aircraft noise. Have for years. And for years, this noise has stressed you intensely - your ears are healthy, the sound is objectively measurable, but somehow it triggers you more and more.
You've also long known: there are official noise assessments claiming this aircraft noise is "no longer harmful after 4 a.m." Even though it's measurable. Even though you can hear it. Even though sleep at 4 a.m. very much still matters - in the early morning hours, sleep gets lighter, so noise wakes you more easily, not less. And what about everyone who works shifts and is only just getting into bed at 4 a.m.?
That has always made you angry - because it isn't just unfair, it's an abuse of power. Expertise being misused to write off people like you - people who can't afford a wealthier neighborhood - as unimportant. Your health is less worth protecting than the health of people in the more affluent part of town.
What you weren't aware of: this experience of injustice had woven itself into your noise reaction and amplified it. Every airplane wasn't just loud - it was also a symbol of being overlooked.
The moment this coupling becomes conscious, something happens: the next jet comes. It's still loud.
It still hurts your ears. But now you can consciously think: "That's the abuse-of-power drama, not the airplane alone." And bit by bit, the unconscious rage about the injustice can be decoupled from the direct reaction to the sound. Your brain relaxes (a little). The gap between stimulus and response becomes visible again - and trainable.
That motivates you to keep going. You notice: okay, if I work on my narratives, on my emotional evaluation of the sound, I can influence the reaction. It doesn't disappear completely, and it's still unfair - but I can deal with it.
This isn't just "thinking positive" - it's reality-testing.
You ask: is this alarm helpful right now? Or is my brain firing for reasons beyond the stimulus alone? And once you can answer that, you have a lever.
So what now?
Are you wondering: okay, but how? If my brain has stored this so deeply - how do I change it?
That's the right question! And the answer is: not with sound training alone.
It's about three levels:
1. Clarify your boundaries Which boundaries matter to you? And more importantly: which ones have others in your life crossed again and again? Not to find someone to blame, but to understand where your nervous system learned to sound the alarm. Misophonia often hides behind an entirely different boundary story.
2. Make your narratives conscious When the chewing sound triggers you, what exactly is it triggering? "This person is ignoring my needs"? Or "I don't matter"? Or "I can't protect myself"? The more specifically you can name the unconscious story, the more you can negotiate with it - like in the aircraft-noise example.
3. Give your nervous system its safety back This is the longer work. It's about your brain relearning: my boundaries are respected. I am not in constant danger. That doesn't happen through thoughts alone, but through real boundary practices and regulation.
This is work that often goes easier with professional support - whether that's therapy, coaching, or structured training. And if your reactions are now significantly restricting your social life: get it checked out. Physically first (an ENT specialist - to rule out hyperacusis or tinnitus, for example), then psychotherapeutically. You don't have to sort this out alone.
But the first step? That one's simple: observe yourself. When does which sound trigger you, around whom? And what exactly is the unconscious message behind it? Once you know that, you have a lever.
